Benefits Available to All Benefit-Eligible Employees 401(k) retirement savings plan Mileage reimbursement Employee Assistance Program (EAP) Paid vacation, sick leave, and holidays Tuition Reimbursement Additional Benefits for Employees Regularly Scheduled 30+ Hours per Week:  Medical, dental, and vision insurance Flexible Spending & Health Savings Accounts Disability, life, and AD&D insurance Pet insurance Pay range: $32 to $48 per hour, depending on experience.    Schedule/Shift : Per diem   Territory/Location: Palm Desert, CA   Responsibilities: Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment.  Develop and implement tailored intervention plans based on assessment findings.  Maintain accurate clinical records for all patients referred to social work services.  Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs.  Serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services.  Qualifications: Master’s degree in Social Work accredited by the Council on Social Work Education.  Minimum of 1 year of experience in healthcare, with preference given to candidates with home health care experience.  Current BLS, hands-on CPR certification.  Valid driver's license and reliable, insured transportation.  Excellent verbal and written communication skills, along with strong organizational abilities.  #LI-Hybrid See what Mission has to offer!
